Job Description:

We are currently looking for a Licensed Clinical Social Worker (LCSW) to join our physician-owned nephrology practice in Reston, VA.

• Outstanding opportunity to join a highly regarded private practice led by a multi-award-winning physician (consistently recognized as a Washingtonian Top Doctor).
• Work alongside our nephrologist, nurse practitioner, and renal dietitian to provide holistic care helping educate patients on chronic kidney disease and manage end-stage renal disease, delivering essential psychosocial support and care coordination in an outpatient setting.
• Key Focus Areas:
  • Assisting patients with insurance transitions, Medicare/benefit counseling, and medication burden management.
  • Providing case management, discharge planning, psychotherapy and psychoassessments, life coaching, and transition of care.
  • Conducting home and virtual visits, and teaching through our CKD education program.
• We prioritize work-life balance!
  • Flexible part-time schedule: 2-3 days per week.
  • No weekends, evenings, or on-call requirements.

Qualifications:

• Education: Master's degree in Social Work required.
• Licensure: Licensed Clinical Social Worker (LCSW) in the state of Virginia is required.
• Experience: Previous experience in nephrology or dialysis social work is preferred, but new graduates are encouraged to apply!
• Must love building relationships with patients, value bedside manner, and possess excellent clinical skills.
• Must comply with HIPAA rules and regulations.
