Mount Vernon Internal Medicine | Remote

Our well-established, physician-owned private practice is seeking a dedicated Nurse Practitioner to join our team in a fully remote, home-based role. We pride ourselves on providing comprehensive outpatient primary care (internal medicine). Quick Look:

  • Schedule: Full-Time | Monday - Friday | Clinic Hours (7 AM - 5 PM). No on-call, evening, or weekend requirements.
  • Remote Flexibility: Rare opportunity for a fully remote, home-based clinical role largely centered around phone outreach and virtual care visits (dedicated office space is available if preferred).
  • Licensure: Active, unrestricted Virginia license is required.

Practice Overview:

  • Broad Scope of Care: Focus primarily on Transitional Care Management (TCM) and proactive quality care outreach for an adult and geriatric patient panel.
  • Clinical Duties: Conduct vital phone outreach for patients recently discharged from inpatient or emergency services. Coordinate follow-up labs and virtual visits for patients following cognitive screenings (Linus tests). Ensure patients are up to date on preventative measures like mammograms, colonoscopies, etc.
  • Supportive Environment: Work collaboratively as a key member of our dedicated Quality Team, alongside a robust clinical network of 12 Physicians and 8 Advanced Practice Providers.

Qualifications:

  • Licensure: Active, unrestricted Virginia license (or eligibility to obtain) is required.
  • Experience: 1+ years of primary care experience preferred. Must be comfortable independently managing telehealth outreach and care coordination.
  • Attributes: Strong bedside manner, a relationship-based approach to care, and HIPAA compliance. A strong work ethic and the ability to connect with patients over the phone are essential.
